What to gather

• Legal facility name, mailing address, and physical address
• Owner, facility contact, and Radiation Safety Officer
• Machine type, manufacturer, model, serial number, tube count, and maximum kVp
• Room floor plan, machine position, and building materials
• Workload and use of spaces beside, above, and below the room
• Installer or registered service provider information

Step 1: Order the shielding review

Prepare the qualified-expert floor plan and site-specific shielding calculations before the room is built when Idaho requires them. Use a qualified expert whose current Idaho status covers the work, and follow the state submission requirements for the facility type.

Step 2: Send the shielding review

Send a copy of the shielding review, floor plan, and calculations to Idaho within 30 days after the facility receives it. Email the program at LabImprovement@dhw.idaho.gov and retain the report with permanent radiation-safety records.

Step 3: Complete the online license application

Enter the facility, owner, contact, and Radiation Safety Officer information. Add every machine and X-ray tube. Submit the application and save the confirmation.

Step 4: Complete the installation notice

The installer or registered service provider submits the online Notice of Installation. The facility should retain the submission confirmation with the machine records.

Step 5: Pay the invoice

Do not send payment with the application. Idaho’s listed standard fee is a $50 facility fee plus $25 for each X-ray tube. The program sends an invoice after processing.

Step 6: Follow the license cycle

Hospitals, clinics, and medical facilities generally renew every two years. Dental, chiropractic, podiatry, and veterinary facilities generally renew every four years. Industrial, research, academic, and security facilities generally use a 10-year cycle.

Step 7: Report later changes

Use the online Change of Information form when facility or machine information changes. Keep the license, machine inventory, shielding report, installation notice, invoice, inspection records, and service records together.

Do not make this mistake

Do not wait until the room is built to order the shielding review. Idaho requires site-specific qualified-expert calculations, and some new facilities need them before licensing.
