Step-by-step instructions

Step 1: Get the shielding design accepted

Email the shielding design to Radiation.DHHS@Maine.gov. Maine will issue an RSA number or acceptance letter. The machine registration will not be processed without it when shielding review is required.

Step 2: Complete HHE-804

Use HHE-804 for the initial fee. A new unit is $60. A replacement unit is $30.

Step 3: Complete HHE-805

List the facility, responsible person, equipment, room, and installation information. Add the shielding acceptance information.

Step 4: Send the forms and payment

Mail the complete package to Maine Radiation Control Program, 286 Water Street, 3rd Floor, Augusta, ME 04333-0011. Keep a copy.

Step 5: Arrange the required qualified-expert inspection

For most clinic, private-office, chiropractic, and veterinary installations, arrange the first qualified-expert inspection on the state's required schedule. Keep every report for later inspection and renewal.
