Step-by-step instructions

Step 1: Complete shielding and equipment checks

Keep the room shielding documentation. Have a Minnesota-registered service provider complete required calibration and performance work before first use.

Step 2: Complete the Initial Registration Form

Use this form only for a new facility, a new owner, or an unregistered satellite location. List the facility and all initial equipment.

Step 3: Mail the form and payment

Mail the application and required payment to Minnesota Department of Health, Radiation Control X-ray Unit, P.O. Box 64497, St. Paul, MN 55164-0497. The annual base fee is $155; equipment fees are added by machine type.

Step 4: Accept the portal invitation
