Step-by-step instructions

Step 1: Finish the room and shielding plan

Complete the layout and shielding calculations before construction or installation. Keep the final report with the registration records.

Step 2: Choose the correct online application

Medical, dental, chiropractic, podiatric, and veterinary facilities use the Healing Arts or Veterinary application. Industrial or other ionizing-radiation devices use the device application.

Step 3: Complete and sign the DocuSign form

Enter the facility, responsible person, machine, and room information. Upload or attach the requested technical records, then electronically sign and submit.

Step 4: Send Form 2579 if available

The installer may send the completed FDA Form 2579 to xrayregistration@msdh.ms.gov or mail it to MSDH Division of Radiological Health, 310 Airport Road, Pearl, MS 39208.

Step 5: Save the completed envelope and registration

Download the completed DocuSign envelope and keep it with the shielding, installation, performance, and state registration records.

The application identifies any current registration charge based on the machine and facility type.
