Step-by-step instructions

Step 1: Plan the shielding

New and relocated Missouri X-ray facilities need a shielding evaluation by a Missouri-approved Qualified Expert. New facilities also need the applicable Qualified Expert radiation-safety survey. Timing depends on the equipment class: CT, fluoroscopy/C-arm, radiation therapy, and mammography must be surveyed before routine clinical use; newly installed dental CBCT must be evaluated and inspected within 30 days; most other new Class B, C, and D equipment must have the required onsite survey within 90 days of beginning use.

Step 2: Complete the registration form

Enter the facility and owner information. List each machine in the inventory section. The form has room for up to 15 machines.

Step 3: Sign and submit

Email the signed form to MRCP@health.mo.gov. You may also mail it to Missouri Radiation Control Program, P.O. Box 570, Jefferson City, MO 65102-0570.

Step 4: Keep the registration file

Save the submitted form, shielding report, installation paperwork, performance tests, and state response.

Step 5: Report later changes

Notify the program when ownership, address, contacts, or equipment change. Follow the state's renewal or update notice when received.
