Step 1: Complete the shielding design and installation survey

Plan the room before construction. After installation, keep the radiation-safety survey for each machine and room.

Step 2: Complete both pages of the registration form

Enter facility, owner, contact, and machine information. Do not skip the second page. Sign the application.

Step 3: Email or mail the PDF

Email the signed form to BXC@dep.nj.gov, or mail it to Bureau of X-Ray Compliance, P.O. Box 420, Mail Code 25-01, Trenton, NJ 08625-0420. Submit within 30 days after installation.

Step 4: Wait for the invoice

Do not send money with the registration form. New Jersey reviews the filing and sends the bill separately.

Step 5: Keep records ready for inspection

Keep the registration, shielding design, radiation-safety surveys, installation documents, and operating procedures at the facility.
