Step-by-step instructions
Step 1: Choose the correct facility application
Use the Rhode Island program page to select dental, veterinary, hospital, specific radiology, general radiology, industrial, or another matching facility type.
Step 2: Prepare the shielding plan and evaluation
Except for regulatory exemptions, new X-ray facilities and modifications require shielding plan review by the Rhode Island Department of Health before construction. The facility application identifies the consulting radiation-physics service and its Rhode Island RPS registration number, so use a provider whose current Rhode Island registration covers the work.
Step 3: Complete every question
Do not leave blanks. Enter the facility supervisor, radiation-protection contact, ownership, addresses, machine inventory, and requested safety information.
Step 4: Add the fee and mail the package
For a Specific Radiology Facility, the current application fee is $225. Other facility types use the fee printed on their form.
Make the check payable to RI General Treasurer and mail to Radiation Control Program, Center for Health Facilities Regulation, Rhode Island Department of Health, 3 Capitol Hill, Room 305, Providence, RI 02908-5097.
Step 5: Keep the registration and report changes
Keep the registration, shielding attachment, inspection documents, and equipment records. A change of ownership requires a new application.
