Step-by-step instructions

Step 1: Complete plan review before construction

New, remodeled, or relocated medical X-ray facilities that are subject to plan review must submit shielding calculations before construction begins or before the machine is moved. WAC 246-225-030 states that diagnostic veterinary, podiatric, and dental facilities are exempt from submitting shielding calculations and floor plans. DOH’s current registrant guidance also lists dental shielding including dental CBCT, diagnostic veterinary, mammography, and podiatry with generators not exceeding 70 kVp among plans it does not review. For a podiatry installation above 70 kVp, confirm the current DOH interpretation before construction. Covered medical rooms must use a qualified expert and Department review.

Step 2: Choose online or mail registration

Online: use Business Licensing Service and add the X-Ray Facility and Devices endorsement. Mail: complete both the Business License Application and the X-Ray Facility and Devices Registration form.

Step 3: Enter the facility and device information

List the location, practice type, machine type, and tube count. Use the fee table in the X-ray form to enter the registration charges.

Step 4: Submit and pay

For mail filing, send both forms and a check or money order payable to Department of Revenue to State of Washington, Business Licensing Service, P.O. Box 9034, Olympia, WA 98507-9034.

Step 5: Keep the annual registration current

Washington bills the facility for re-registration every year. Keep the business license, X-ray registration, plan-review letter, shielding calculations, and inspection records.
