Step-by-step instructions

Step 1: Complete the shielding design

Prepare the room and shielding plan before construction or installation. Keep the final report for inspection.

Step 2: Complete the registration form

List the facility, owner, contacts, and every X-ray tube. Submit within 30 days after acquiring the machine.

Step 3: Calculate the fee

Add the facility administrative fee to $65 for each X-ray tube. Administrative fees are $130 for chiropractic, dental, podiatric, and veterinary facilities; $940 for hospitals; and $455 for other facilities.

Step 4: Mail the form and full payment

Mail the package to DEP Bureau of Radiation Protection, P.O. Box 8469, Harrisburg, PA 17105-8469. Use a check or money order payable to Commonwealth of Pennsylvania.

Step 5: Prepare for inspection and renewal
